Please check box on the left: Retreat Waiver and Release of Liability- I hereby acknowledge that I have voluntarily chosen to participate in the retreat organized by Amalia Moscoso. In consideration for being permitted to participate in the retreat, I hereby agree to the following: Assumption of Risks: I understand that participating in the retreat involves certain risks, including but not limited to physical injury, emotional distress, and property damage. I voluntarily assume all such risks and agree to hold harmless Amalia Moscoso, its organizers, instructors, and any associated personnel from any and all claims, liabilities, or damages arising from my participation in the retreat. Physical and Mental Health: I certify that I am physically and mentally fit to participate in the retreat. I understand that it is my responsibility to inform the organizers of any medical conditions, allergies, or physical limitations that may affect my ability to safely participate in any activities. Release of Liability: I hereby release, waive, and discharge Amalia Moscoso, its organizers, instructors, and any associated personnel from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in the retreat, including without limitation, negligence of any kind or nature. Indemnification: I agree to indemnify and hold harmless Amalia Moscoso, its organizers, instructors, and any associated personnel from any loss, liability, damage, or cost they may incur due to my participation in the retreat. Photography and Video Release: I hereby grant Amalia Moscoso the right to use any photographs, videos, or other media taken during the retreat for promotional, advertising, or other purposes without compensation to me. Acknowledgment of Understanding: I have carefully read and fully understand the contents of this waiver and release of liability. I acknowledge that I am voluntarily agreeing to its terms and that I am not relying on any oral or written representations or statements made by Amalia Moscoso other than what is set forth in this waiver. I acknowledge that by checking off this waiver, I am giving up substantial legal rights, including the right to sue Amalia Moscoso.
